Notes to Applicant:

1. This application is for full-time Higher Nitec and Nitec students of ITE Colleges who passed at least one core module, but left the course from 1 Jan 2009 without completing it successfully.

2. Module Certificate is issued for application made within 12 months from the date of leaving ITE course.

3. Applicant whose leave date is more than 12 months can apply for Certified Statement. Application form for Certified Statement can be downloaded at http://www.ite.edu.sg/admission/exams/services.htm (administration fee of $10 is applicable).

4. Processing of application may take up to 2 weeks from the date of application. Applicant will be informed via email to collect the Module Certificate once the Certificate is ready.

5. As the Module Certificate is issued only once with no replacement, applicant is required to collect the Certificate at ITE HQ Customer Service Centre (http://www.ite.edu.sg/admission/exams/services.htm) during office hours (between 8:00am and 6:00pm from Monday to Friday except public holidays). If applicant is authorising another person to collect, the person must bring an authorisation letter duly signed by applicant, a copy of the applicant’s NRIC and the authorised person’s NRIC.

6. Module Certificate must be collected within 1 month after notification. Certificate not collected will be disposed based on prevailing policy.
