Healthcare is an important segment in Singapore’s ageing population today. Join this course to expand your knowledge in the area of chronic illness management and learn to provide quality care to patients.

This program will give you an understanding of the needs of this group of patients and give you the necessary skills to improve the quality of life for people with chronic illness.

What you'll learn

  • Conduct basic health assessment
  • Initiate care plans for clients with chronic conditions
  • Apply various chronic disease models in care delivery
  • Provide palliative care to clients with chronic diseases
  • Perform health promotion and prevention
  • Provide general health education
  • Integrate care between acute and long-term care sectors, including community care services

Entry Requirements

  • Graduates from Nitec in Nursing with a valid Singapore Nursing Board (SNB) practising certificate

Note: Applicants applying for this course must have a valid practising certificate issued by the Singapore Nursing Board. 

Progression Opportunities

  • Higher Nitec in Chronic Diseases & Integrated Care graduates may apply for progression to the first year of a related diploma course at the Polytechnics. 
  • Higher Nitec in Chronic Diseases & Integrated Care graduates may apply for progression to related ITE Work-Study Diploma (previously known as Work-Learn Technical Diploma).

Career Opportunities

Graduates from Higher Nitec in Chronic Diseases & Integrated Care can look forward to employment opportunities in the industry as Nursing Staff in acute hospitals, community hospitals or nursing homes.
